主题: Hospital Responses to a Diagnosis-Based Payment Scheme with a Global Budget in China

Abstract:

This paper investigates how hospitals responded to a policy reform in China in 2016 that switched the payment scheme from fee-for-service to a diagnosis-based payment scheme with a global budget. We find that hospitals responded to the reform on multiple dimensions, including upcoding, adjusting the composition of medical services, admitting more cases, and altering the patient composition across diagnostic categories. Across each dimension, we observe substantial heterogeneity in hospital responses based on their characteristics. We do not find any negative impacts of the reform on patient access to healthcare services or patient health outcomes.

张川川，浙江大学经济学院研究员（百人计划文科A1类）、博士生导师，浙江大学劳动经济研究所副所长，国家级人才计划青年学者（2022）。张老师长期从事微观经验研究，特别是使用自然实验方法和观察数据进行政策评估，研究内容聚焦于养老、医疗、教育和就业等民生领域，尤其关注老弱病幼贫等弱势群体的福利，在推动国内应用经济学研究发展和服务国家需求方面发挥了积极作用。他合作或独立完成的部分研究工作发表于《中国社会科学》《经济研究》《经济学季刊》《管理世界》《金融研究》等国内经济学、管理学、金融学权威期刊，以及Demography, AEJApp, JDE, JCE等人口学和经济学国际权威期刊，发表的中文论文被《新华文摘》《中国社会科学文摘》《高等学校文科学术文摘》等重要刊物多次转载。目前担任China & World Economy副主编（2021.05-），《财贸经济》执行编委等学术兼职。
